Orofacial myofunctional concerns can involve patterns of the tongue, lips, jaw, and face. An evaluation can help determine whether therapy, medical assessment, dental care, or another next step may be appropriate.

OMDs may include forward or low tongue posture, tongue thrust swallowing, open-mouth resting posture, mouth breathing patterns, oral habits, chewing differences, or restricted oral tissues. Causes are often multifactorial: learned patterns, airway barriers, allergies, enlarged tonsils or adenoids, dental development, oral habits, and structural differences can all play a role.
When these patterns persist, they may be associated with speech sound errors, swallowing or chewing differences, dental or orthodontic concerns, and sleep or airway concerns that require medical evaluation. Myofunctional therapy focuses on functional patterns — tongue and lip posture, nasal breathing support after airway issues are addressed, swallowing, and speech placement when appropriate.
After an evaluation, recommendations may include therapy, home practice, monitoring, or coordination with dentists, orthodontists, physicians, or other professionals when clinically appropriate and authorized. Frequency and duration vary.
Signs of an orofacial myofunctional disorder
- Mouth is frequently open at rest
- Tongue rests against or between the teeth
- Visible tongue push when swallowing
- Difficulty with certain speech sounds (s, z, sh, ch)
- Messy eating or difficulty chewing
- Thumb sucking continuing past age 4
- Snoring or noisy breathing during sleep
- Orthodontic relapse after braces
